When you make an appointment, I will email the forms to you or click just on "FORMS button" below so that you can download and bring them to your first session. If you are unable to receive the forms in advance, please plan on arriving 15-20 minutes early to your scheduled initial appointment so that you can fill out the paperwork prior to your first visit. I will review these forms and the information you provide at that time, as well as answer any additional questions you may have.
